You may also pay in person with cash, check or credit card when you arrive for your appointment.

If your appointment is covered by your insurance plan I will bill Aetna (Washington only) or provide you with a form for reimbursement from a different insurance company.

The fees below are discounted and apply only when paid before or at time of service. If paying after service is provided, standard fees will apply ($180 initial appointment, $128 follow-ups).
